Membership Forms

We have made available both our Membership Application and Direct Debit forms for you to down load and fill in. You can return the completed forms by either mail, fax or in person to the club. The forms are available in both Adobe Acrobat PDF format and Microsoft Office Word format.

Please read the forms carefully before completing them and returning to them to the club. Boths forms contain important information relating to Membership at Waratah Golf Club.
